What is First Link?

First Link is a partnership program with health care professionals and the Alzheimer Society of Thunder Bay to ensure that individuals and families dealing with dementia receive support and information throughout the course of the disease. First Link partners include family physicians, geriatric specialists, pharmacists, social service workers in health facilities and community agencies and staff working in family practice.

Here's What You Do

Ask your patient or their family member’s permission to forward his/her name to the Alzheimer Society of Thunder Bay. Complete the referral form and let us know by fax, email or phone that you would like us to contact them.
