Thank you so much for choosing to support Alzheimer Society of Thunder Bay. Your generosity will allow us to provide support, counselling and information to individuals and families living with Alzheimer’s and to fund research to find a cure for the disease.

The type of donation you are making

The name and address of the family or friend you would like to be notified of this donation (if applicable)

Your income tax receipt will be mailed to you within 4 weeks. If you would prefer to receive an e-tax receipt please let us know.

Please ensure that your name and address are printed clearly

Mail the form to our office along with your credit card information or a cheque made payable to Alzheimer Society of Thunder Bay (please do not send cash in the mail)
